OFFICE BILLING POLICY

Patients With Dental Insurance:

Preventative Procedures: Cleaning & Fillings:   Our office will gladly submit a claim for services rendered assuming proper information is supplied.   Once insurance payment is received, a bill will then be sent out for the balance. 

Major Procedures:   All major procedures will be preauthorized with your dental insurance company prior to treatment.  After we receive your preauthorization you will be contacted by our office to inform you of your coverage and the amount you are expected to pay the day of service.

Emergency Treatment:  25% of the dental fee is expected the same day of service.

Patients Without Dental Insurance:

For patients without dental insurance, payment is expected at the time of service.

Major & Endodontic Therapy Procedures: (ex.  Crowns, Bridges, Implants, Root Canals):  50% of the dental fee is due the day of service and the remaining 50% is due at the second or final visit of the procedure. 

For single visit root canal treatments, full payment is required the day of service. 

PAYMENT PLANS:  Available for all patients

We understand the financial limitations of our patients and realize that in some cases, payment in full is not an option.   Our payment plan options are as follows:

- On balances of $150 or less, the balance is due the day of service.  We accept Visa, MC, or Discover. 

-On balances of $150 or more,  payment is due the day of service.  If you are unable to pay your balance at that time, we will help you set up a 3, 6, or 12 month, interest free health credit card through CITI MASTERCARD.  Brochure is available at the front desk.  Or you can apply online at http://www.healthcard.citicards.com/ .  ( If credit is denied, payment is due in full at time of service. )
